As of May 30, 2023 all Representative Claims forms for Minor Children will require the “Ontario- Attestation of Personal Representative” form to be completed. These forms are available in hard copy at the Mohawks of the Bay of Quinte Administration Building (24 Meadow Drive). Failure to submit this form will result in the default being applied to the claim and the Minor Child payment to be submitted to the Ontario Superior Court to be held in trust until the Minor Child reaches the age of majority.
