Each eligible (registered MBQ member over the age of 12) individual within a household should complete the survey. Kindly note that survey responses will only be included from MBQ members over the age of 12.

When completing the survey, please answer as many of the questions that you can. If you cannot, or are uncomfortable answering any question, please leave it blank and move on to the next question. If you would like to change a response, please do so using the back button within the survey.
